General Summary

We are holding a block of rooms and will be making a reservation with the Aloft Chicago O'Hare hotel upon receipt of this registration form. The cost of the meeting is $795 which includes attendee meals and meeting costs.  The cost for spouses' dinner and reception is $100 each.  The room rate has been established at $159 per night. 

The session will begin with a Welcome Dinner/Reception at 7:00 pm on Sunday, September 16th and will end at 3pm on Tuesday, September 18th.  Please plan to make your departure flight for 5pm or later on the 18th. 

All meeting fees will be charged if there is a cancellation less than a week before the session.
